Our palliative care focus is centered around treatment at home for chronic illness or disease. We support you by helping you to manage your symptoms. The goal is to improve your quality of life, every day. Also to help prevent any unnecessary pain and suffering.
In coordination with the hospital palliative care team, in Victoria, we can provide additional personal care services that help you manage your symptoms and also provide valuable feedback to your health team.
What is the difference between palliative care and hospice care? Palliative Care is provided to those patients that are managing illness or disease with treatments or comfort care, but are not necessarily terminal illnesses. This same care is deemed Hospice Care when the client has less than six months of life remaining.
Many people are unaware that hospice at home is a possibility. You have the right to choose where you wish to be at end of life. Our team bring their own personal experiences, that help them to empathize with the grieving process, as well as, embrace it. Hospice Care is all about comfort and quality of life in the remaining days, or months. It is a time that you can be enjoying your friends and family in the comfort of your own home. We work with your Hospice team to help create an environment free from pain and suffering.
Understanding the importance of both palliative and hospice care helps to ensure the seriously ill receive the right level of care at the right time so they can make the most of each day that they have.
